If you are out of range for at-home phlebotomy, you can go to any Quest Diagnostics location for your blood draw. You can find your requisition form in the "documents" section of the Geviti app or web portal. Simply bring a digital or hardcopy of this form to any Quest lab location for your blood draw, no appointment needed. If you would like an appointment, you can schedule one here.
If you live in NY or NJ, you must go to Bioreference instead of Quest Diagnostics. You cannot go to a Quest in NY or NJ. If you are unsure which lab to go to, look for the lab name at the top of your requisition form.
